INFORMATION AND EXPRESSION OF AGREEMENT REGARDING THE PROCESSING OF PERSONAL DATA

By signing this form, I agree that My Hotel Apartments will process the personal data from the CHECK-IN form legally, fairly and in a transparent manner towards me and will take all necessary guarantees to ensure respect for my right to privacy. The above personal data will be processed for the purpose of concluding and executing the accommodation services contract and fulfilling the related legal obligations and involve the following operations:
- making the reservation and sending the confirmation
- preparing legal financial and accounting documents and fulfilling other legal obligations related to the services provided
- sending the satisfaction questionnaire

I have been informed that, to the extent that I do not provide the above personal data, Continental Hotels SA will not be able to perform the obligations specific to the contract concluded with the undersigned. I have also been informed that these data will be processed by the authorized personnel of My Hotel Apartments and may be transmitted based on legal obligations or legitimate interest, as the case may be, to various authorities, courts. The above personal data will be processed during the execution of the hotel services contract and subsequently for periods in accordance with the legal obligations of My Hotel Apartments (financial and accounting documents - 10 years, registration sheets - 5 years, statistical reporting - 5 years, etc.) depending on the categories of data processed.

According to Regulation No. 679 of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, I have taken note of the rights I enjoy as a data subject, namely:
- right of access
- the right to data portability
- the right to rectify or delete data
- the right to object to the processing of personal data concerning me
- the right to request restriction of data processing
- the right to address the judiciary and the National Supervisory Authority for the Processing of Personal Data
I declare that I have taken note that the personal data concerning me will not be subject to an automated decision-making process (profiling)

I have taken note of the fact that in order to exercise these rights I can submit/send a written request, dated and signed, to the attention of the "Personal Data Protection Officer", to the e-mail address myhotelapartments@gmail.com or to the Company's address at Strada Constantin Dobrogeanu Gherea 64, sector I Bucharest Romania.

GENERAL TERMS AND CONDITIONS FOR PROVISION OF ACCOMMODATION SERVICES

The accommodation services are provided by the Company with the Client's compliance with the "house rules" which can be consulted in the room, at the reception or on the website www.myhotelapartments.ro . The following rules are of particular importance and are imperative:
- It is forbidden to carry out activities in the room or within the hotel premises that disturb the rest of the guests (noise, indecent behavior/language, etc.)
- According to law 349/2002, smoking is prohibited in enclosed public spaces. Our hotel will charge a cleaning fee of 500 lei if you smoke in this room.
- The hotel is not responsible for uninsured valuables.
- Check-in is from 2:00 PM. Rooms must be checked out before 11:00 PM.

PAYMENT TERMS AND GUARANTEE

The Client personally guarantees payment for the services provided by the Company, both accommodation and other additional services.
